Dear parents and caregivers
Thank you to those parents and caregivers who have completed a 2024 medication form.
All students that are required to have medication administered by school staff at school must complete a new medical form for 2024.
Medical forms must be updated at the beginning of each school year.
Staff will not be able to administer medication without a 2024 medical update. 2023 medication that was not collected at the end of 2023 must be picked up as soon as possible.
Please bring in your child's updated details to the office and the office staff will be able to assist you.
Please ensure the medication has clear Doctor instructions.
